Employers should complete this form and return it to our office so we have the necessary information to prepare your payroll journal. If we prepare your payroll this form is mandatory; otherwise, you may complete it at your option.

Georgia employers with more than ten employees on January 1st of each year are required to register and participate in the E-Verify program. As of December 31, 2018, Florida private employers are not required to participate; however, bills have been introduced that may make all employers participate in E-Verify.
